Activity and Engagement Documentation

A comprehensive guide to documenting meaningful activities, social engagement, and wellbeing outcomes to demonstrate person-centred care.

Why Activity Documentation Matters

Meaningful activity is fundamental to wellbeing and is a key element of CQC's "Responsive" domain. Good documentation demonstrates that activities are person-centred, shows the impact on resident wellbeing, and provides evidence of quality of life beyond basic care needs.

Life History and Preferences

The foundation of meaningful activity is understanding each resident:

Past Interests

  • Hobbies and pastimes
  • Career and work life
  • Family role and traditions
  • Places lived and travelled
  • Significant life events

Current Preferences

  • Favourite activities now
  • Group vs individual preference
  • Best time of day for engagement
  • Physical capabilities
  • Cognitive abilities

Recording Activity Participation

For each activity session, document:

  • Activity name and type
  • Date, time, and duration
  • Who facilitated the activity
  • Residents who participated
  • Individual engagement level (observed, participated, led)
  • Enjoyment/response indicators
  • Any adaptations made for individual needs

Measuring Engagement and Wellbeing

Go beyond simple attendance records to document impact:

Mood before and after

Note any visible change in mood, facial expressions, or verbal feedback

Level of participation

Passive observer, active participant, or leading/helping others

Social interaction

Conversations initiated, connections made, friendships observed

Verbal feedback

Resident's own words about the activity in quotes

Physical indicators

Smiling, laughing, animated conversation, physical engagement

Types of Activities to Document

Structured Group Activities

  • Arts and crafts sessions
  • Music therapy and sing-alongs
  • Exercise classes and movement sessions
  • Quizzes and games
  • Religious services and spiritual activities
  • Entertainment and external visitors

Individual Meaningful Activities

  • 1:1 time with staff (conversation, life history work)
  • Personal hobbies (reading, knitting, puzzles)
  • Sensory activities (hand massage, aromatherapy)
  • Reminiscence sessions
  • Technology use (video calls, tablets)

Community and Family Engagement

  • Family visits and outings
  • Community group visits
  • Intergenerational activities
  • Trips and outings
  • Involvement in home life (helping with tasks)

Residents Who Decline Activities

Document respectfully when residents choose not to participate:

  • Offer made and response
  • Reason given (if any)
  • Alternative offered
  • Pattern of declining (time of day, activity type)
  • Alternative engagement that was accepted

Respecting Choice

Residents have the right to decline activities. Document offers and responses without judgement. Focus on finding what the individual does enjoy rather than recording refusals negatively. Some residents prefer solitary activities, which are equally valid.

Activity Care Plan

Each resident's care plan should include:

  • Life history summary highlighting interests
  • Current activity preferences
  • Activities that bring joy
  • Best approaches for engagement
  • Physical/cognitive considerations for activity adaptation
  • Goals related to activity and engagement
  • Family involvement preferences

Evidence for CQC

Activity documentation provides evidence for multiple CQC domains:

Responsive

  • Person-centred activities based on preferences
  • Adapted activities for individual needs
  • Variety of activities offered

Caring

  • Dignity and respect in activity provision
  • Emotional support through engagement
  • Meaningful relationships facilitated

Effective

  • Wellbeing outcomes documented
  • Activities supporting care goals
  • Multi-agency working (therapists, volunteers)

Well-led

  • Activity programme planning
  • Staff training in engagement
  • Monitoring and improving activity provision

Activity Programme Review

Document regular review of the overall activity programme:

  • Participation rates by activity type
  • Feedback from residents and families
  • Gaps in provision identified
  • New activities introduced based on resident preferences
  • Staff training needs for activity delivery
