What Are Time-Critical Medications?
Time-critical medications are those where the timing of administration directly affects their effectiveness or safety. Delayed or missed doses can cause rapid deterioration, medical emergencies, or reduced treatment effectiveness.
Parkinson's: The Most Time-Critical
Parkinson's medications are among the most time-critical of all. Delays of even 30 minutes can cause severe symptoms including inability to move, speak, or swallow. The Parkinson's UK "Get It On Time" campaign emphasizes this is a patient safety issue.
Parkinson's Disease
Medications
Levodopa (co-beneldopa, co-careldopa), dopamine agonists
Timing Window
Within 30 minutes of scheduled time, ideally to the minute
Consequences of Delays
Delayed doses cause rapid deterioration: freezing, rigidity, falls, swallowing difficulties, inability to communicate
Note: Timing is more critical than for almost any other condition. Even 30-minute delays can cause significant problems.
Epilepsy
Medications
Anti-epileptic drugs (phenytoin, sodium valproate, levetiracetam, etc.)

Within 2 hours of scheduled time
Consequences of Delays
Missed or late doses increase seizure risk. Some AEDs have narrow therapeutic windows.
Note: Maintain consistent timing. Never stop AEDs suddenly. Brand switching can affect control.
Diabetes (Insulin)

All insulin types
Timing Window
Must coordinate with meals. Fast-acting: with meals. Long-acting: consistent daily time.
Consequences of Delays
Hypoglycaemia or hyperglycaemia depending on timing error. Can be life-threatening.
Note: Never give insulin without food unless specifically prescribed. Always check blood glucose.
Infections (Antibiotics)

Specific antibiotics (some require precise timing)
Timing Window
As prescribed - some every 6 hours, some every 8 hours, etc.
Consequences of Delays
Delayed doses reduce effectiveness, may contribute to antibiotic resistance.

Exact times, often 12 hours apart

Systems for Ensuring Timely Administration
Identification
- Flag time-critical medications on MAR charts/eMAR
- Include in care plans with specific timing requirements
- Highlight during handover
- Use visual alerts (stickers, colour coding)
Process Changes
- Schedule medication rounds to accommodate time-critical medications
- Consider separate "priority round" for critical medications
- Set alarms/reminders for specific times
- Ensure adequate staffing at critical times
- Have contingency plans for staff absence
Documentation
- Record exact time of administration (not round time)
- Document any delays and reasons
- Report patterns of delayed administration
- Review timing as part of regular medication audits
