Medication Administration Record

Also known as: MAR, medication administration record

The record of the medicines prescribed for a resident and every administration against them, historically kept on paper and increasingly kept electronically.

A MAR chart (Medication Administration Record) lists the medicines prescribed for a resident and provides a space to sign for each administration. Standard codes record when a medicine is given, refused, withheld, or unavailable.

Paper MAR charts are still common, but they are easy to leave blank, hard to audit, and give no live prompt when a dose is missed. Electronic records (see eMAR) address those gaps.

Why it matters for care homes

Gaps and unexplained codes on a MAR chart are a frequent finding at inspection. Accurate, complete records are the primary evidence that medicines are handled safely.
