Person-centred care
Also known as: person centred care, personalised care
Care that is shaped around the individual's needs, preferences, history, and goals, rather than around a fixed routine.
Person-centred care treats the person as an individual and builds their care around what matters to them: their preferences, relationships, history, culture, and goals. It involves the person, and where appropriate their family, in planning and reviewing their own care.
It is a core expectation of good adult social care and a theme the Care Quality Commission looks for throughout its assessment, particularly under the Caring and Responsive key questions.
Why it matters for care homes
Person-centred detail in care plans and daily records is what distinguishes genuinely individualised care from generic documentation, and it is visible to inspectors and families alike.
