Care plan
Also known as: care and support plan, support plan
A working document that records a resident's assessed needs and exactly how the home will meet them, reviewed and updated as things change.
A care plan is a written record of a resident's assessed needs and how those needs will be met, covering areas such as personal care, mobility, nutrition, medicines, and social and emotional wellbeing.
It is a living document. Good practice is to review it regularly and whenever the person's needs change, and to keep it consistent with the daily records of care actually delivered.
Why it matters for care homes
Care plans that are out of date, generic, or contradicted by daily notes are a common inspection concern. Current, person-centred plans are central evidence of effective, responsive care.
